Hospital discharge

A number of people told us that they are concerned about how patients are discharged from hospital and whether provisions are in place for people once they arrive back in their own home or in a nursing home.

We spoke to hospital and PCT leads about the current processes and we also gathered patients' own experiences. We have now compiled a report to feedback to the organisations.
